29 CFR 1926.502(h)(1)(iii): The safety monitor wan not on the same walking/working surface and within visual sighting distance of the employee being monitored:    a) United Roofing & Sheet Metal, Inc. on or about June 26, 2014, at the site of the Education Building on the Bowling Green State University Campus, Bowling Green, OH: The employer failed to ensure the designated safety monitor was in a position to observe all employees operating on the roof deck. Due to the configuration of the roof profile (penthouse perimeter) the designated monitor was unable to actively observe all employees staging material throughout the work area. Employees working outside the direct view of the monitor were exposed to a 70 ft. fall to the ground below.
