29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a) 3474 Frankford Avenue, Philadelphia, PA - Employees performing dismantling tasks of swing-type suspended scaffolding within 1 foot of the unprotected edge were exposed to a fall hazard of approximately thirty (30) to thirty-five (35) feet without the use of a guardrail system, a safety net system, or personal fall arrest systems.  On or about 27 JUNE 2014.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) 3474 Frankford Avenue, Philadelphia, PA - The employee, utilizing a fiberglass portable extension ladder, extended the ladder from the lower roof to the upper roof level of the structure.  However, the ladder's length did not allow the ladder to extend enough to allow 3 feet above the upper roof landing level.  Observed on or about 27 JUNE 2014.
