29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) South Side - On or about June 26, 2014 employees were exposed to impalement hazards while working near or passing near upright reinforcing steel. Protective caps or other devices were not used to protect employees from impalement hazard.
