(b) On or about June 23, 2014, and at times prior, employees were exposed to a fall of approximately 12 feet to the ground below framing trusses between the first and second level of the new residential home.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: (a) On or about June 23, 2014, and at times prior, employees were exposed to a fall of approximately 21 feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards. (b) On or about June 23, 2014, and at times prior, employees were exposed to a fall of approximately 12 feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
