29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: (a) Southwest Corner of Building - On or about June 26, 2014 an employee was exposed to a fall hazard while working. Employee was elevated 13 feet above the ground on a pallet. The pallet platform was not attached to the forks or structure of the forklift.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) Southwest Corner of Building - On or about June 26, 2014 an employee was exposed to a fall hazard while standing on a wood pallet elevated 13 feet above the ground by a rough terrain forklift. Employee was not protected from falls by the use of a personal fall arrest system or guardrail system.
