29 CFR 1910.180(d)(4)(vi): Complete inspection(s) of crane(s), including the requirements of paragraph (d)(3) of this section, were not performed at intervals as generally defined in 1910.180(d)(2)(ii) of this section with a careful examination and determination made as to whether any deficiencies constituted a safety hazard, i.e., load, boom angle, and other indicators, e.g., the RCI (Rated Capacity Indicator) and RCL (Rated Capacity Limiter), over their full range, for any significant inaccuracies: (a) On the concrete yard-slab where Hornbeck Supply Vessel C-200 was being assembled - On or about June 25, 2014, employees were exposed to injury due to the wired, lower load cell sensor on the main hoist line cable of the North Manitowoc 16000 Crawler Crane, #2241, not being fully operational or accurate; this load cell was re-installed after having been repaired, but it was not load tested before crane #2241 was put back into service on 6/23/14, nor thereafter. Consequently, the RCI and RCL could not have properly sensed the load and alerted the operator to overload conditions.
