29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a)  At Lot 84 Bellaton Subdivision Daphne, Alabama:  On or about June 25, 2014 and at times prior; the employer allowed his employee to work from a work platform that only measured 12 inches wide exposing the employee to a fall hazard of 14 ft to the concrete floor below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At Lot 84 Bellaton Subdivision Daphne, Alabama:  On or about June 25, 2014 and at times prior; the employer failed to ensure his employees involved in framing activities had fall protection in place while they were working from the ceiling joists exposing them to a fall hazard of 19 feet to concrete ground below.  b)  At Lot 84 Bellaton Subdivision Daphne, Alabama:  On or about June 25, 2014 and at times prior; the employer failed to ensure his employees involved in framing activities had fall protection in place while they were working from walk board exposing them to a fall hazard of 14 feet to concrete ground below.

29 CFR 1926.1053(a)(1): Ladders shall be capable of supporting the following loads without failure:  a)  At Lot 84 Bellaton Subdivision Daphne, Alabama:  On or about June 25, 2014 and at times prior; the employer exposed his employees to fall hazards in that he allowed his employees to use a job made ladder on site to gain access to and from the second level of the house which did not have proper blocks installed.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At Lot 84 Bellaton Subdivision Daphne, Alabama:  On or about June 25, 2014 and at times prior; the employer failed to ensure a job made ladder used on site by employees to gain access to and from the second level of the house had rails that extended at least three feet past the landing area exposing his employees to a fall hazard of 11 feet to the concrete floor below.
