On or about June 27, 2014 - at the above addressed jobsite, employees performing roofing operations approximately fifteen (15) feet eight (8) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.    CITYWIDE HOME EXTERIORS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 424387, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 31, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 6647 CONWAY COURT, OAK FOREST, IL  60452.
