On or about June 16th, 2014, at the above addressed job site, employees were not protected from falling from a 12 feet high concrete block wall. REPEAT: COMMERCIAL CONCRETE SYSTEM LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 836584.015 , CITATION NUMBER 1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 9, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 400 GULF SHORE BLVD. NAPLES, FL 34102
