29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Machine Shop - On or about June 25, 2014, authorized employee(s) were required to change saw blade(s) and/or adjust the arbor die on the automatic shear. Hardware (lockout / tagout device) for the 1 1/4 ball valve was not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Employee(s) were thereby exposed to the hazards associated with inadvertent startup of equipment.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Machine Shop - On or about August 6, 2014, the employer had not documented through a written certification that periodic inspections had been performed on the lockout/tagout procedures (automatic shear) in the company's energy control program.
