29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: M & G Utility Construction, Inc., Fort Lee, Monroe Manor: On or about 06/24/14, an employee was observed in a trench excavation that was 4'7" deep without the use of a ladder for a means of egress. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

M & G Utility Construction, Inc., Fort Lee, Monroe Manor: On or about 06/24/14, an employee was working in an excavation 4'7" deep while the excavated material was not at least 2 feet from the edge. Abatement certification required within 10 days after abatement date.
