29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about June 24, 2014, at the above addressed job site, employees were exposed to the hazard of falling, while doing tar/roofing work on a commercial building at a height greater than 13 ft. without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about June 24, 2014, at the above addressed job site, employees were exposed to the hazard of falling, while using a portable extension ladder which was not extended 3 ft. above the landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   a) On or about June 24, 2014, at the above addressed job site, employees were exposed to the hazard of falling while using a portable extension ladder which was not secured from displacement.
