On or about April 15, 2014, and at times prior thereto, employees at the Artisan Hotel used an electrical flexible cord with a metal box that was not provided with GFCI in construction demolition work, were the safety of the equipment was not determined on the basis of suitability of the equipment for the purpose identified. This condition exposed employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about April 15, 2014, and at times prior thereto, employees were using an energized flexible cord connected to a receptacle located in the Artisan Hotel first floor that was not provided with a strain relief device; so that it will prevent flexible cord electrical wires from the terminal screws to be pulled. This condition exposed employees to electrical hazards.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. On or about March 25, 2014, and at times prior thereto, employees at the Artisan Hotel were permitted to start demolition operations without previously conducting an engineering structure survey by a competent person. This condition exposed employees to struck-by, crush and building collapse hazards.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work is started. In each case, any utility company which is involved shall be notified in advance. On or about March 28, 2014, and at times prior thereto, employees at the Artisan Hotel were performing demolition operations at 11th and 12th floor without shutting off 120VAC electrical circuits. This condition exposed employees to electrical hazards.

On or about march 15, 2014, and at times thereto, employees used a portable grinder located in the first floor by the entrance to the Artisan Hotel that was not equipped with safety guards.
