29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a.  On or about June 30, 2014, on the south side of the roof, an employee working beneath employees installing roof shingles was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a.  On or about June 30, 2014, on the south side of a flat roof, three employees installing shingles on the roof of a commercial building were not protected from a 14 feet 2 inch fall hazard by the use of a personal fall protection system.
