Card Heating and Air Conditioning Services, Inc.; 5151 Buffalo Road, Erie, PA - On or about 6/9/14, the company did not report the death of an employee on a job site that occurred on May 29, 2014, due to a heart attack.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   A.)  Card Heating and Air Conditioning Services, Inc. - On or about, 6/9/14,  employees did not receive training on the new labeling elements and safety data sheet format by the December 1, 2013 deadline noted in the 29 CFR 1910.1200(j)(1) of the Hazard Communication -2012 Standard.  Employees work with or around chemicals that included but were not limited to refrigerant, degreaser, and oil.
