On or about June 30, 2014 - at the above addressed jobsite, an employee performing restoration operations from a GENIE S-80 telescopic aerial boom lift worked from the mid rail of the boom basket, thereby exposing the employee to fall hazards. No abatement certification or documentation is required for this item.
