29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a.  On or about June 30, 2014 - at the above addressed jobsite, employees performing roofing operations approximately twenty four (24) feet three (3) inches above the ground were not protected from falls, thereby exposing the  employees to fall hazards.  b.  On or about June 30, 2014 - at the above addressed jobsite, employees conducting roofing operations were exposed to falls to the inside of the building created by the presence of unprotected skylights.    No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.  On or about June 30, 2014 - at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface nor was it secured at its top, thereby exposing the employees to the hazards associated with falls.   No abatement certification or documentation is required for this item.
