29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:      a) Sheet-fed Press room    On 6/17/2014, while checking outflow of product from Rapida 205 5 color press, pick up pump joint failed splashing aqueous coating Q1540B  into employee's face and eyes.    Condition occurred on or about 6/17/2014.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Sheet fed press room    The employer did not conduct a periodic inspection of their energy control procedures to ensure they properly implement such procedures during lockout of equipment, including but not limited to KBA Rapida 205, 205UV, 105 and 162A printing presses.        Violation occurred on or about 6/26/2014.
