Instance 1) Employees were exposed to fall hazards greater than 6 feet at an unguarded second floor balcony, located at the top of the stairs. Instance 2) An employee fell greater than 6 feet from the attic stairwell opening to the second floor below, where the floor opening did not have a guardrail.
