29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a.)  Raider Clinic Building 525, Fort Myer: On or about 06/24/14, a safe means of access was not provided to the employee working on a scaffold platform in that employee had to use the scaffold cross braces to gain access, as a result employee was exposed to a 7' fall hazard.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure to control or minimize those hazards:   a.)    Raider Clinic Building 525, Fort Myer: On or about 06/24/14, an employee working on the platform of a fabricated frame scaffold 7' from the ground was not trained in the hazards associated with scaffolding, as a result employee used the cross braces to access the scaffold platform.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
