29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 1 July 2014, and at time prior thereto, two workers were cutting pieces of lumber and handing them up to workers above or were performing framing activities, exposing the workers to a struck by hazard from falling materials, tools, and debris.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On or about 1 July 2014, and at times prior thereto, a worker was using a pneumatic nail gun to perform framing activities while a second worker was using a hand-held circular saw, both without the use of eye protection, exposing them to the hazard of being struck by flying objects.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about 1 July 2014, and at times prior thereto, a hand-held circular saw was being used to cut pieces of lumber while having strain relief damage, and also being used with an extension cord with similar cord damage, exposing the worker using it to an electrical hazard of approximately 120 volts.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   On or about 1 July 2014, and at times prior thereto, a worker was performing framing activities along the ridgeline of a residence under construction at the roof of the second story without fall protection, exposing the worker to a fall hazard of approximately 9.5 to the second story floor below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about 1 July 2014, and at times prior thereto, a worker was using a 6 foot fiberglass stepladder to access a second story top plate by using the top step, exposing the worker to a fall hazard of approximately 9.5 feet to the second story floor below.
