29 CFR 1910.333(a)(2): Employer did not ensure employee was familiar with the policy and procedures of working around live parts that were not de-energized, and/or safety-related work practices were not used to protect employee. Employer's work practices were not suitable for the conditions under which the work was being performed and for the voltage level of the exposed electric conductors or circuits: On or about June 28, 2014, the employer failed to provide suitable procedures to ensure that live parts were de-energized prior to beginning work on a 30 amp MA4 circuit #9 on the south side of mud pit. Employee(s) were exposed to electrical hazards while performing routine work assignments.
