29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a) A portable ladder used by employees to access and egress the roof had structural defects, such as, missing slip-resistant feet. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 20 feet above the ground. Zeo Group, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 974921, citation number 1, item number 1 and was affirmed as a final order on May 8, 2014, with respect to a workplace located at 1333 Warrington, Deerfeld, IL 60015. Zeo Group, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 961033, citation number 1, item number 1 and was affirmed as a final order on May 8, 2014, with respect to a workplace located at 9 Acacia Dr., Hawthorn Woods, IL 60047. Zeo Group, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 959184, citation number 1, item number 3b and was affirmed as a final order on May 8, 2014, with respect to a workplace located at 7 Acacia Dr., Hawthorn Woods, IL 60047. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
