29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) Fabrication Shop: On or about June 19, 2014, and at times prior thereto, an employee required to wear a half face elastomeric North respirator, to sweep dried granite dust, had several days of facial hair growth that interfered with the seal of the respirator. (b) Fabrication Shop: On or about September 26, 2014, and at times prior thereto, an employee required to wear a half face elastomeric North respirator to dry cut small pieces of granite, had several days of facial hair growth that interfered with the seal of the respirator. Note: Abatement certification required for this item.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) Fabrication shop: On or about June 19, 2014, and at times prior thereto, each employee who operated the Daewoo G25S-2 (SN: 10-01259) fork lift to move large pieces of granite from the yard to the shop, had not had refresher powered industrial truck training within the past three years. (b) Facility: On or about June 19, 2014, and at times prior thereto, each employee who operated the Komatsu F-G25T-12 (554508A) fork lift to move packages and supplies from the delivery truck to the storage space, had not had refresher powered industrial truck training within the past three years. Note: Abatement certification required for this item.
