29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about July 1, 2014 and times prior thereto, at the workplace located at or near 3201 Falling Water Road, Sherwood, Arkansas, employees were observed throwing pieces of wood from the ground level to a worker on a ladder approximately 14 feet in height above the ground below without a helmet or any other type of personal protective equipment for the head. This condition exposed employees to struck by hazards.

This condition exposed employees to the hazard of falls from an elevation.
