29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    a. At the Three Rivers Community College construction site, during masonry work on July 1, 2014, an employee worked on the tubular welded frame scaffold with no end rails, on the southwest corner of the building on the third tier of the scaffolding.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a. At the Three Rivers Community College construction site, during masonry work on July 1, 2014, an employee walked onto a pallet to retrieve bricks; the pallet, raised by a forklift, was positioned outside the scaffold guardrails.

Employees were not provided chemical hazard communication training, such as for the acid wash used at the Three Rivers Community College construction site; employees were not aware of the contents or hazards of the acid wash.
