29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: a) Job site, 12 Fourth Street, Herminie, PA: On or about July 1, 2014, during service/maintenance work activities on machinery, the employer did not an ensure that an appropriate energy control program was in use. Deficiencies noted during the inspection included, but were not limited to, performing set up work or insert changes on CNC units with the power on, changing out lathe chuck heads with no lock on the disconnect, changing blades on the Do All band saw without following proper energy control procedures and not providing energy control training for employees for employees during these types of activities.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instances including, but not limited to the following: a) Job site, 12 Fourth Street, Herminie, PA: On or about July 1, 2014, during production activities on the Roll In Bandsaw, unit number 218, located in the lathe department of building number 1, the employer did not ensure that machine guarding was provided over the unused portion of the bandsaw blade. b) Job site, 12 Fourth Street, Herminie, PA: On or about July 1, 2014, during production activities on the Bridgeport Mill, serial number 01-18-0171, located in the lathe shop of building number 1, the employer did not ensure that machine guarding was provided. c) Job site, 12 Fourth Street, Herminie, PA: On or about July 1, 2014, during production activities on the Baldor sanding machine, catalog number CL3503, located in the CNC department of building number 1 , the employer did not ensure that machine guarding was provided over the nip points of the sanding belt and pulley system on the unit. d) Job site, 12 Fourth Street, Herminie, PA: On or about July 1, 2014, during production activities on the Mazak QT 15 CNC unit, of building number 1, the employer did not ensure that machine guarding was provided over machinery parts while an employee was hand sanding parts in the machinery as it was running. e) Job site, 12 Fourth Street, Herminie, PA: On or about September 4, 2014, during production activities on the Kalamazoo belt sander unit, located in the main production area of building number 2, the employer did not ensure that machine guarding was provided over the nip points of the sanding belt and pulley system on the unit. f) Job site, 12 Fourth Street, Herminie, PA: On or about September 4, 2014, during production activities on the Cincinnati sheer, located in the main production area of building number 2, the employer did not ensure that machine guarding was provided over the hold down clamps. g) Job site, 12 Fourth Street, Herminie, PA: On or about August 4, 2014, during production activities on the Milwaukee Drill press, located in the main production area of building number 2, the employer did not ensure that machine guarding was provided over the rotating chuck head and drill bit. h) Job site, 12 Fourth Street, Herminie, PA: On or about August 20, 2014, during production activities on the Do All band saw, located in the main production area of building number 2, the employer did not ensure that machine guarding was provided over the unused portions of the blade.
