29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) On or about June 26, 2014, , the employer directing the work on the job site did not conduct frequent inspections that would provide a workplace free from recognized hazards where any employee, worker, laborer or mechanic was employed in the performance of their work. a) workers performing masonry activities worked from tubular welded scaffold platforms that were not fully planked; were not equipped with end guard rails, and did not have an attached ladder for egress and access exposing them to unprotected fall hazards; and b) workers working at lower levels and from the ground below the tubular welded scaffold were not provided or wearing protective hard hats exposing them to head injuries from falling and flying objects.
