29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: At the Cuero High School Stadium bleachers, a SKYTRAK powered industrial truck was parked with a load of support beams on its forks under the bleachers. Employee were unloading these beams from the lift. Employees were not wearing protective helmets (hard hats) to protect against striking their head on steel beams on the underside of the bleachers and were not provided with gloves to protect their hands while carrying the 4 ft. long metal beams.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. At the Cuero High School Stadium bleachers, this powered industrial truck was parked with a load of support beams on its forks. An employee was unloading these beams from the lift. The powered industrial truck used for this construction operation had worn tires exposing the inner tube of this tire/wheel system and possibly exposing employees working in the vicinity to struck-by hazards from a fallen/shifted load.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder: At the Cuero High School Stadium bleachers, an employee was walking down a Werner Telescoping ladder while the ladder was in the closed position leaning against the bleachers. The employee was using the ladder, folded and leaning against a bleacher system, as a form of stairs, exposing himself to a fall.
