29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) On or about July 02, 2014, employee(s) were exposed to fall hazards up to approximately 25 feet in height to the ground level below, when working with no fall protection on the steep roof having an 8/12 and 12/12 pitch, while painting the exhaust stack pipes on the roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. (a) On or about July 02, 2014, employees were exposed to falls of approximately 12 feet in height to the ground level below, when using a Werner extension ladder extended 12 inches above the landing, while climbing to the roof to paint the exhaust pipes.
