29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway were not kept free from protruding nails, splinters, holes, or loose boards. On or about 07/01/2014, At the seventh floor of the B Mill Cleaning House, a stairway used to gain access to a bin top, had an uneven landing surface caused by a missing bolt. This condition created tripping hazards for employees who use the stairway.
