29 CFR 1910.119(d)(3)(i): The employer did not compile information pertaining to the equipment in the process: L&H Packing facility, San Antonio, Texas: On July 1, 2014, and times prior thereto, the employer did not compile complete and accurate information representative of the ammonia refrigeration process as follows: a) Process safety information for V-6 is missing and/or not included in the employer's equipment files. b) The King air unit is installed in the field, but is missing from the current P&IDs. c) OER-R3 VRT freezer recirculator descriptive information is not available on the P&IDs. d) V-16 chiller oil pot detail is not included in the employer's equipment files. e) Pressure relief valves are not individually identifiable - several PRVs installed in the facility have identical serial numbers.
