29 CFR 1910.23(a)(8): Every floor hole into which persons could accidentally walk was not guarded by either a standard railing with standard toe-board on all exposed sides, or a floor hole cover of standard strength and construction.  While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing:   Wash Rack area: A ground hole which measured 2 1/2 inches by 21 inches was not covered to prevent employee injuries.  Batch Operations area: A sump pump hole was left uncovered exposing employees to trip hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Shop: A Delta table drill press did not have a guard to prevent employee contact with rotating chuck and drill bit and a Makita cut-off saw had a non-functioning guard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   Mechanic Shop:  Flexible cord connected to an outlet was used to supply power to a wall mounted quadruplet receptacle outlet box through a hole at the top of the wall mounted quadruplet outlet box.  Various pieces of electrical equipment were energized from the quadruplet receptacle outlet at different times.
