On 6/17/2014, the owner and four employees from Daniel Bacquec LLC were working on the roof of a residential building located at 8108 Glen Arbor Drive, OFallon MO.  They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The owner and employees were observed on the roof of the building without fall protection, exposing them to fall to the ground below.
