At the worksite where employees were using an extension cord to provide power to a circular saw, the ground prong was missing from the extension cord, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. In between building 14 and 20 where employees laid an extension cord across a roadway, the extension cord was not protected from vehicular damage, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. At the worksite where employees were using an extension cord to provide power to a circular saw, the strain relief of the male plug was pulled away from the outer insulating jacket of the extension cord, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. At building 20 where an employee was working from the elevated basket of an aerial lift, a fall protection was not used, thereby exposing the employee to a 19 foot fall hazard.
