29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a. At the worksite where employees used a blue extension cord with a missing ground prong and damaged outer insulation jacket to provide power to circular saws, the cord had not been inspected before the days use, thereby exposing employees to an electrical contact hazard.  b. At the worksite where employees used an arrange/black extension cord to provide power to circular saws, the cords strain relief was pulled away from the outer insulation jacket on both ends, having a damaged female plug, and damage to the outer insulation jacket, thereby exposing employees to a contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a. At the temporary receptacle where employees plugged in an extension cord to provide power to circular saws, the ground prong was missing from the male end of the blue extension cord, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. At the worksite where employees used extension cords to provide power to circular saws, the strain relief was pulled away from both the male and female ends of the blue extension cord exposing any tension to the terminals, thereby exposing employees to an electrical contact hazard.  a. At the worksite where employees used extension cords to provide power to circular saws, the strain relief was pulled away from both the male and female ends of the orange/gray extension cord exposing any tension to the terminals, thereby exposing employees to an electrical contact hazard.
