29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 3, 2014, in the production area, a radial drill was not guarded, where operators were exposed to lacerations and crushing hazards at the rotating drill shaft and point of operation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. On or about July 3, 2014, the employer did not correctly enter the following work-related injury on the OSHA Form 300 or equivalent for the calendar year 2014: Case No. 3, occurring on June 30, 2014, in which an employee experienced a cut and fracture to the left hand while operating a drill press. As of July 3, 2014, the employee had not returned to work, but the injury was not classified as Days Away From Work on the OSHA 300. Abatement Certification and Documentation is required
