On or about July 1, 2014, at the above addressed jobsite, an employee conducting water installation in a trench five (5) feet seven (7) inches in depth did not have a safe means of egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about July 1, 2014, at the above addressed jobsite, employees were installing a water line in a trench measured at five (5) feet seven (7) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
