29 CFR 1926.453(b)(2)(v): A body harness was not worn and a lanyard attached to the boom or basket when working from an aerial lift. East side of the building; an employee applied TEX Clean masonry cleaner to the building structure while working from a JLG Lift, model 6042, TX105121 without the use of fall protection, exposing employees to fall hazards at approximately 12 feet.
