29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent.   Monitored employees in the following areas exceeded the action level:  A) An employee in Structural/Middle Bay was exposed to continuous noise of 96.8 A-weighted decibels (dBA) or an equivalent dose of 258.3% for 452 minutes on 8/5/2014. Exposure calculations include a zero increment for the 28 minutes not sampled.  B) An employee in Structural/Bay 3 was exposed to continuous noise of 92.7 dBA or an equivalent dose of 145.2% for 445 minutes of 8/5/2014.  Exposure calculations include a zero increment for the 35 minutes not sampled.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program.  Employer had not conducted noise dosimetry monitoring to determine employee time-weighted average exposure for employees working in the shop/fabrication area(s). OSHA exposure monitoring on 8/5/2014 indicated employee(s) in multiple locations were exposed to time-weighted average noise levels exceeding 85 decibels.

29 CFR 1910.107(b)(5)(i): Visible gauges or audible alarms or pressure activated devices were not installed to indicate or insure that the required air velocity is maintained.  Manometers for the facility paint spray booth were not properly connected and were not functioning during the inspection. Air velocity in the spray booth was measured at 19-21 linear feet per minute. Employee(s) were potentially exposed to hazardous paint during parts spraying operations in the booth.
