29 CFR 1910.305(g)(1)(iv)(A): Flexible cords were being used as a permanent source for wiring: a. Maintenance Shop - The employer did not provide permanent wiring as two relocatable power taps, that were series connected, were used as a substitute for permanent wiring in the maintenance area. A cable television set top box was plugged into one of the relocatable power taps which was plugged into a second relocatable power tap mounted on the wall, exposing employees to electrical shock. Observed On or About June 27, 2014.
