29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Jobsite, 362 Argon Street, New Kensington, PA: On or about July 3, 2014, during roofing activities, the employer did not ensure that the ladder, used to gain access to/egress from the roof, was extended at least 3 feet above the roofline or that it was properly secured and that a grasping device was provided where fall hazards of approximately 12 feet existed.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways as necessary, that enabled each employee to recognize hazards related to ladders and stairways, and did not train each employee in the procedures to be followed to minimize these hazards:   a) Jobsite, 362 Argon Street, New Kensington, PA: On or about July 3, 2014, during roofing activities, the employer did not ensure that ladder safety training was provided for employees at the work site where fall hazards of approximately 12 feet existed.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   a) Jobsite, 362 Argon Street, New Kensington, PA: On or about July 3, 2014, during roofing activities, the employer did not ensure that fall protection systems/devices were used where fall hazards of up to approximately 12-14 feet existed.    Boneco, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 680158, citation 1, item 1, and was affirmed as a final order on December 7, 2012, with respect to a workplace located at 2100 Leishman Avenue, Arnold, PA.
