29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite, 362 Argon Street, New Kensington, PA: On or about July 3, 2014, during roofing activities, the employer did not ensure that the ladder, used to gain access to/egress from the roof, was extended at least 3 feet above the roofline or that it was properly secured and that a grasping device was provided where fall hazards of approximately 12 feet existed.

Boneco, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 680158, citation 1, item 1, and was affirmed as a final order on December 7, 2012, with respect to a workplace located at 2100 Leishman Avenue, Arnold, PA.
