29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) Ramp - On or about 06/03/2014, workers were exposed to a struck-by hazard from unstable soil which had not been scaled off or otherwise removed from the side of a concrete block wall. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
