29 CFR 1910.26(c)(3)(i): The angle of the ladder was not proper in that distance from the base to the vertical wall was not equal to one-fourth the working length of the ladder.  a)  Work Site: 11 West 7th Street, Bayonne, NJ  Employee engaged in air conditioner installation work, utilizing an aluminum extension ladder, was exposed to a fall hazard of approximately 15 feet to the ground below due to improper placement of the ladder.   Violation occurred on or about 7/2/2014.

29 CFR 1910.26(c)(3)(iii): Ladders were used on surfaces which did not provide a secure footing:  a)  Work Site:  11 West 7th Street, Bayonne, NJ  Employee installing an air conditioner unit was exposed to a fall hazard of approximately 15 feet to the ground below as the aluminum extension ladder being utilized was placed on grass and did not have secure footing.   Violation occurred on or about 7/2/2014.

29 CFR 1910.26(c)(3)(vii): Portable ladders were used  for other uses than that which they were intended.  a)  Work Site:  11 West 7th Street, Bayonne, NJ  An aluminum ladder which did not extend to sufficient height to perform work was utilized thereby exposing employee to an approximate 15 foot fall to the ground below.   Violation occurred on or about 7/2/2014.
