29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:     (a)  Right Side Scaffold - On or about July 1, 2014 employees were exposed to a fall hazard due to a scaffold not being fully decked or planked. One deck was not installed on the scaffold resulting in a 36 inch gap. Employees would fall 11 feet 4 inches onto rocky surface.

(a)  Right Side Scaffold - On or about July 1, 2014 employees were exposed to a fall hazard while standing on a scaffold 11 feet 4 inches above rocky soil with no base-plates installed.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.  (a)   Left Side and Right Side Scaffolds - On or about July 1, 2014 employees were exposed to a fall hazard while working on a scaffold that was missing cross-braces on the backside.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.     (a)  Scaffold - On or about July 1, 2014 employees were exposed to a fall hazard while from a scaffold with a guardrail system or personal fall arrest system. Employees were exposed to falling 11 feet 4 inches to a rocky surface.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.     (a)  Roof - On or about July 1, 2014 employees were exposed to a falling up to 15 feet to a lower surface while working on a low-slope roof with no fall protection.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable:   (a)  Construction Site - On or about July 1, 2014 employees were exposed to a fall hazard while working on scaffolds without training on how to recognize, eliminate and prevent fall hazards.
