29 CFR 1910.178(l)(2)(i)(B): A forklift trainee was not prohibited from operating a powered industrial truck not under the direct supervision of a trainer and where such operation could endanger the trainee or other employee: (a) Facility - On or about June 26, 2014 a powered industrial truck trainee was operating a Crown TR3600-200 tugger in an unsafe manner while moving a rack of tires exposing himself or others to struck-by or crushing hazards.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). (a) Facility - On or about June 26, 2014 employees operated Crown tuggers that are in need of repair. Tuggers were operated with missing top door plates which cover the steering, electrical and drive units of the tugger.
