29 CFR 1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6.      a) Cylinder was not secured to prevent movement, in that the cylinder was free standing not chained to the wall to prevent from being tip over.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) The company did not develop procedures, implement training and periodic reviews to address control of hazardous energy when performing servicing and maintenance of equipment such as but no limited to changing saw blades.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    a) Powered industrial vehicle operators did not receive formal and operational training , in order to fully evaluate their competency to operate including powered industrial truck such as but not limited to sit-down forklifts.

29 CFR 1910.303(g)(1)(i)(B): The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels; and      a) The area in front of the electrical disconnect box was not maintained to gain access to the box, in that the panel was blocked by a refrigerator.
