29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:  a)  Kittanning Citizens Bridge, Kittanning, PA, on or about July 2, 2014 - The employer did not ensure that employees using a tight fitting full facepiece respirator were fit test prior to the use of that respirator.  Employees were utilizing 3M 6899 B full face respirators during spraying operations and were not fit tested prior to the use of that respirator.  b)  Kittanning Citizens Bridge, Kittanning, PA, on or about July 2, 2014 - The employer did not ensure that employees were fit tested with the same respirator facepiece model that was used.  An employee was utilizing a 3M 6200 respirator but was fit tested with a 3M 7500 series respirator.
