29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: On or about July 3, 2014 and in times prior thereto a Miller derrick belt was in use and was equipped with tool rings that could be mistaken for positioning attachments. The waist belt was for work positioning only and the tool rings were not intended to support a person. The D ring in the upper back section of the harness was the only ring designed for fall protection and the employer did not ensure the employee hooked it to the fall protection anchorage. The fall hazard was located in the derrick of Drilling Rig #4.

The derrick man was not trained as to the hazards of hooking off to the tool rings that were not designed for fall protection. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one hour: On or about July 3, 2014 and in times prior thereto oxygen, acetylene, and propane cylinders were stored together, located in the lower doghouse. This condition exposed workers to a fire hazard at the Oil Well Drilling Rig#4 location.
