29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) SBC Building 11, On or about September 12, 2014 - Employees who preformed scrap torch burning with acetylene and liquid oxygen on carbon alloy metals were not provided sleeves, and/or arm coverings to adequately protect employees from second and third degree burns.

29 CFR 1910.178(o)(1): Stable or safely arranged loads were not handled and caution was not exercised when handling off-center loads which cannot be centered: a) In Building 11, On or about August 13, 2014 - A bundle of carbon steel coil approximately 41.1 feet long in the shape of a trapezoidal was lifted and driven by forklift to the storage location approximately 20 feet down the center of the aisle. The load was not stable as it was handled by the forklift.
