29 CFR 1910.178(o)(1):  Stable or safely arranged loads were not handled and caution was not exercised when handling off-center loads which cannot be centered:    a) In Building 11, On or about August 13, 2014 - A bundle of carbon steel coil approximately 41.1 feet long in the shape of a trapezoidal was lifted and driven by forklift to the storage location approximately 20 feet down the center of the aisle.  The load was not stable as it was handled by the forklift.
